Guiding Statement + Objectives:

Co-creating a space where lived experiences on heart health social (in)justices can be openly shared and discussed among community and academic audiences

Objective 1 →   Engage with C2DREAM research and activities on improving heart health and reducing chronic diseases across Minnesota.

Objective 2 →   Learn about approaches to build collaborative and respectful research relationships that reflect the strengths and assets of communities.

Objective 3 →   Learn how C2DREAM research frames the link between social and structural determinants and inequities in heart health and chronic disease.

Conference Venue

The conference will be held in the event space. There will be signage indicating to walk through the restaurant space of Clyde Iron Works.

Do I need to register for this event?

  • Yes, registration is required to attend this event.

Is there a registration fee for this event?

  • No cost! C2DREAM is proud to host its intended audience of community members, youth, researchers, policymakers, and clinicians at no charge to them.

Will lunch be provided?

  • Yes! Lunch, a snack and all day beverage service will be provided by C2DREAM. Please ensure that you have completed your registration form if you have any dietary restrictions.

What if I have dietary restrictions?

  • C2DREAM will try our best to accommodate dietary restrictions. Please ensure they are listed in your registration form.

I am traveling to Duluth and want to stay overnight. Is there a room block available?

  • Yes! C2DREAM has a block of rooms set up at the Radisson Hotel Duluth-Harborview that is just 10 minutes from the venue. Book your room at the Radisson using this link: z.umn.edu/C2DREAMHotelBlock.

When does registration close?

  • In order to remain within our venue and catering requests, registration will close the third week of May. The end of the day Friday, May 24. If you are interested in attending the conference after registration has closed, please contact C2DREAM@umn.edu

  • Many of the conference details are still being finalized. Please check here closer to the event for details regarding CEUs.

What if I register and can’t attend?

  • Please email C2DREAM@umn.edu as soon as you are able. An email for final confirmation of attendance will be sent to all registrants 2 weeks prior to the conference.

What is the preferred attire for the conference?

  • We ask that you attend wearing business casual.

c2dream annual health equity conference

The goal of the C2DREAM annual health equity conference is to bring together community and academic audiences to learn more about cardiovascular disease inequities in Minnesota. The conference facilitates shared learning between community and academic partners and builds networks to address health inequities in CVD.

In addition, attendees will learn about capacity building and strategies for community-based participatory research. This conference will facilitate collaboration, bi-directional knowledge sharing, and capacity building for cardiovascular health equity research in Minnesota.

Each year, the conference will travel to one of our institutional locations: Duluth, Twin Cities and Rochester, MN.

2nd annual health equity conference

The 2nd annual conference took place in Rochester, MN on May 23, 2023. About 130 people attended, consisting of researchers, community members, city and state officials, and more. The conference consisted of riveting panels, information sessions and the crowd-favorite poster session that included three “People’s Choice” awards that were voted for by conference attendees.
