Establishing collaborative partnerships is essential for developing, conducting, and disseminating research on the prevention, treatment, and management of chronic condition outcomes. To support the success of the goals of C2DREAM, a proactive infrastructure is needed. This is co-led by a coalition of expert regional stakeholders, who lead and develop processes that disseminate information to improve healthy hearts for all.
Building strong foundations for future leaders .
The Community Engagement (CE) Core’s overarching goal is to support the activities of C2DREAM by building a strong foundation of collaboration and partnership needed to meaningfully address the shortage in chronic disease care and knowledge for people in Minnesota. The CE Core will:
1. Support the activities of C2DREAM by building a regional community and academic Coalition to offer guidance and input on teamwork and dissemination.
2. Build a strong foundation of engagement practices across disciplines in academic and community stakeholders.
3. Guide dissemination throughout C2DREAM.
4. Evaluate CE Core activities using RE-AIM.
The CE Core will generate sustained impact by creating an innovative infrastructure for the dissemination of research on the prevention, treatment, and management of cardiovascular disease and the related chronic conditions of hypertension and obesity for people in Minnesota.
how we do community engagement
The Community-Centered Dissemination Toolkit is a resource designed to help research teams incorporate collaborative principles into dissemination. The Toolkit was created between the University of Minnesota Medical School Program in Health Disparities Research (PHDR) and Clinical and Translational Science Institute’s (CTSI) Community Engagement to Advance Research and Community Health (CEARCH) along with the help of community members, faculty, staff, and students at the University of Minnesota.
Sarah Gollust, Professor in the School of Public Health and Co-Investigator of the Community Engagement Core, created an introductory video on the Community Centered Dissemination Toolkit. Sarah Gollust was part of the team creating the Community-Centered Dissemination and is part of C2DREAM to support dissemination efforts. The video was created for staff and faculty in C2DREAM to support their efforts in designing dissemination plans and serve as a helpful introductory resource to dissemination for those outside of C2DREAM.
